Near Pure Evils who suffer from some sort of mental illness, serious mental disorders, and an unstable mental state. Despite being mentally ill, they sometimes have an understanding of ethics, morality and a conscience and thus, have moral agency.

Mentally ill characters usually exhibit some abnormal behavior or a mindset that may differ greatly from other people. This can sometimes give them sympathetic qualities if their life has been made harder due to these issues or if they have suffered some form of tragedy that caused them to develop a mental illness.

While psychopathy and mental illness are completely different concepts as the latter is a personality disorder loosely connected to mental illness itself, the vilest of these kinds of villains possess psychopathic traits and tendencies and often engage in antisocial behavior (e.g., Baek Jun Ki, Yoshikage Kira, Sabretooth, and Prometheus, ).

Examples are:

  • Esdeath, who suffers from sadistic personality disorder along with obsessive love disorder.
  • Baek Jun Ki, who has hemophobia and PTSD due to the death of his mother and younger sister.
  • Wrath, who is shown to have an intermittent explosive disorder.
  • Hantengu, who suffers from schizophrenia and delusional disorder.
  • Lord Shen, who suffers from chronic paranoia and anxiety on being abandoned and unloved by his parents.
  • Yoshikage Kira, who has obsessive-compulsive personality disorder and cheirophilia.
  • Lydia Rodarte-Quayle, who has OCD.
  • Sally Acachalla, like her mainstream counterpart has an extreme low intelligence due to being infected with a zombie virus.
  • Sabretooth, who suffers PTSD flashbacks to when his father abused him as a child.
  • Jiho Park, who has schizophrenia.
  • Sergey Taboritsky, who also suffers from schizophrenia.

NOTE: Please do NOT add Psychopaths/Sociopaths to this category as they are completely sane and fully aware of reality, whereas mentally ill villains are not completely aware of reality and have limited views on it and do not have full control over their behaviour. If a character has psychopathic traits but also suffers from mental illness, they would fall under mentally ill instead. And do not add psychotic villains to this category, as this is a subcategory for psychosis, only add them if they show signs of psychosis.
